Is California Tap Water Fluoridated? The Law, the Levels, and How to Filter It Out If You Want To
Fluoride is one of the few water-quality questions where California homeowners get genuinely inconsistent answers depending on which city they live in — and, increasingly, where the science itself is being publicly debated rather than settled. Some California utilities have added fluoride to their water for 70 years; others have never added it at all and never will. Here's what the state actually requires, how to find out what's in your own tap, what the current research says on both sides, and what it takes to filter fluoride out if that's the choice you want to make for your household.
Does California require fluoridated water?
Yes, but only for larger systems, and only when the money is available for it — which has made the requirement far less universal in practice than most homeowners assume. California's fluoridation law (Health & Safety Code §116409, enacted as AB 733 in 1995) requires any public water system with 10,000 or more service connections to fluoridate its water once outside funding for the equipment and ongoing cost is secured, typically through a grant from the California Department of Public Health or a private foundation. Smaller systems and private wells are never required to fluoridate.
That funding condition is why fluoridation status varies so much city to city even among large California utilities. Los Angeles and San Diego add fluoride to their water; San Francisco has fluoridated since the early 1950s; other sizable systems have gone decades without securing the funding to install equipment, and some smaller cities have never fluoridated by local choice. There is no single answer for "California" as a whole — it depends entirely on your specific water provider.
How to find out what's in your water
Every public water system in California is required to publish an annual Consumer Confidence Report (CCR, sometimes called a water quality report) listing fluoride and other measured contaminants — most utilities post the current and past years' reports on their website, or you can call and ask directly. If your home is on a private well, there's no CCR to check: fluoride occurs naturally in some California groundwater, at levels that vary by geology and can run both below and above the level added intentionally by municipal systems, so a private well is only reliably tested, not assumed. What the research actually shows — both sides
This is a case where the honest answer is "the evidence supports real benefits at the levels used in U.S. water systems, and a separate body of newer evidence has raised a legitimate question about exposure well above those levels" — both things are true at once, and conflating them is where a lot of the public confusion comes from.
On effectiveness, a 2024 Cochrane systematic review by Iheozor-Ejiofor and colleagues — one of the most rigorous evidence syntheses in medicine — analyzed decades of water fluoridation studies and found that community water fluoridation meaningfully reduces cavities in children's baby and permanent teeth, and that it remains cost-saving compared to the dental treatment costs avoided (study on Google Scholar). That's the basis for the CDC and American Dental Association continuing to recommend it.
On the other side, a January 2025 systematic review and meta-analysis in JAMA Pediatrics by Taylor and colleagues at the National Institutes of Health — building on a 2024 National Toxicology Program monograph — pooled dozens of epidemiological studies and reported an association between higher fluoride exposure and lower measured IQ in children (study on Google Scholar). It's important to read that finding precisely rather than as a blanket verdict on U.S. tap water: the authors themselves note that most of the strongest associations came from studies where exposure was at or above 1.5 mg/L — roughly double the 0.7 mg/L level the U.S. Public Health Service recommends for community water systems — and that the evidence at typical U.S. fluoridation levels was more limited and uncertain. That distinction is exactly why the finding has triggered new scientific and political debate (including changed federal recommendations and a handful of states and cities ending fluoridation in 2024-2025) rather than an outright consensus reversal.
For a homeowner, the practical takeaway isn't picking a side in that debate — it's knowing what your household's actual exposure level is (via your CCR or a well test) and deciding, with that number in hand, whether you want it reduced.
If you want to reduce or remove fluoride
Not every filter does this. Standard carbon filters — the kind built into most pitcher filters, refrigerator dispensers, and basic under-sink cartridges — remove chlorine taste and sediment but do essentially nothing for fluoride, because activated carbon doesn't bind to fluoride ions. The two treatment types that reliably work are:
